I confirm that I am qualified to deliver services independently and am responsible for ensuring appropriate quality standards in line with this licence stamp including professional membership to my Governing Body

I understand that I must notify Clear Sky should there be a change in my registration status which affects my ability to deliver services to children and families. (Clear Sky reserves the right to decline registration if we are notified of such changes to your circumstances)
